Simple past tense:

subject + V2 + O

ex:
(+) I went to primary school in England.
(-) My grandmother didn't like cats.

Simple present tense:

Subject + V(use s at the back if the subject are he, she, it) + O

ex:
(+) She wears jeans all the time.
(-) He doesn't enjoy it.
(?) Does Jack smoke?

1) Simple Present
(+) S + V1(s/es) + O/C => She reads a book.
(-) S + do/does + not + V1 + O/C => She doesn't read a book.
(?) Do/does + S + V1 + O/C? => Does she read a book?

2) Simple Past
(+) S + V2 + O/C => We cook a cake.
(-) S + did + not + V1 + O/C => We didn't cook a cake.
(?) Did + S + V1 + O/C? => Did we cook a cake?
